I'm transferring to a new server and I'm unsure on the following fields for the MySQL Configuration. Are they all relating to the destination databse and server or are some of them the source database? The wiki says the first and fourth are the new one, but doesn't make clear whether the second and thrid are the old username and password or the new one.
MySQL Server
MySQL username
MySQL password
MySQL database
Also re File transfer and configuration.
Is the Ftp server a number like 94.136.40.103, a url or localhost or /public? How do I find out?
Is the path /public_html or something like tha?

As specified in our restore wiki, the mysql details refer to the new restore database, so they need to be created prior to using them.
Same for ftp details, but you can skip those if you are unsure what to use and leave the default options checked.
